Treatment of Multiligament Injuries

Multiligament knee injuries involve damage to two or more ligaments, often accompanied by meniscal tears. These are complex cases that require specialized surgical management. Dr. Nikolaos Sachinis is an expert in both open and arthroscopic procedures for the reconstruction of multiligament injuries. Arthroscopic surgery in such cases is particularly demanding due to the proximity of major blood vessels and nerves, requiring high precision and advanced surgical skill.

Examples of surgical procedures include:

  • Arthroscopic reconstruction of the posterolateral corner (PLC) of the knee is a technically demanding procedure due to the close proximity of the sciatic nerve and surrounding neurovascular structures.
  • Repair of posterolateral corner (PLC) injuries is ideally performed within three weeks of the trauma. Within this timeframe, direct suturing of the damaged structures is often feasible. If this window is missed, reconstruction with a larger surgical incision and tendon grafts is typically required, as the tissue quality and healing potential decrease over time.
  • Repair of Ramp-type meniscal tears, which often accompany anterior cruciate ligament (ACL) injuries, is crucial for restoring knee stability. Addressing these hidden tears during ACL reconstruction significantly reduces the risk of future ligament failure and the need for revision surgery. Early arthroscopic identification and repair improve long-term outcomes and joint function.

    In cases of medial collateral ligament (MCL) rupture, instability occurs on the inner side of the knee. Dr. Sahinis utilizes personalized grafts—such as the quadriceps tendon instead of the semitendinosus—for enhanced stability and strength. Additionally, in rarer cases involving meniscal root tears, he performs arthroscopic repair using a transtibial fixation technique. This method ensures a secure and effective outcome while preserving as much of the meniscus as possible, avoiding extensive tissue removal.
